Welcome to the Black v. Smith Transport, Inc. Online Claim Form submission page.
If you are an individual who was notified of the Data Breach by letter from Smith Transport, and you wish to sign up for credit monitoring and identity protection services or had out‐of‐pocket expenses or lost time spent dealing with the Data Breach, then a claim form should be filled out online or submitted by mail. You may receive a payment if you fill out a claim form, if the settlement is approved, and if you are found to be eligible.

Claims forms must be submitted online or postmarked by July 17, 2025.
To start your online claim form, you must login with your Unique ID and PIN. You can find your Unique ID and PIN at the top of your postcard Notice. If you are unable to locate your ID & PIN, please contact us by emailing info@SmithTransportSettlement.com. Please request your Claim Login ID & PIN by providing the Class Member’s full name and mailing address.
